The document contains several essential notices that employees should review carefully:

Premium Assistance Programs (CHIPRA/CHIP Notice): Employees and dependents who are eligible for Medicaid or CHIP may qualify for premium assistance programs that can help pay for employer-sponsored health coverage. Indiana residents can contact the Health Insurance Premium Payment Program at 1-800-403-0864 or visit in.gov/medicaid for more information.

Women's Health and Cancer Rights Act: The Lake County health plan provides benefits for mastectomy-related services, including all stages of reconstruction, prostheses, and treatment for complications. Contact the plan administrator at 219-755-3211 for details.

Wellness Program: The Lake County Government Blood Draw is a voluntary wellness program available to all employees, administered in accordance with federal regulations including the ADA and HIPAA.
